Transcript Request Process

Transcript Request Form (Attendees and Graduates)

Please submit a completed Graduate Transcript Request Form (link below) and $10 fee for each transcript requested. The checks or money orders should be made out to “Thunderbird Adventist Academy.” You may also call the main office at (480) 948-3300 to pay by credit card over the phone and fax the request to (480) 443-4944. Upon receiving your request and payment, expect between 3 – 5 business days to fulfill the request. During holidays and August it may take longer than 5 business days to fulfill the request.

You may choose the option of picking the transcript up during regular business hours, or request the transcript to be sent or faxed to an institution, such as a college or employer. Please make sure that the institution will accept faxed copies of the transcript, since many will not. Transcripts picked up in person will be presented in a sealed envelope. If the envelop is opened or the seal tampered with, the transcript can no longer be considered official.
